VIA Health Partners is a community-based, not-for-profit hospice and palliative care provider driven by compassionate care. We are growing and seeking registered nurses to coordinate patient/caregiver care, perform assessments, and implement plans of care with a focus on education and support across the care team.

Responsibilities include visits, triage by phone and in person, and collaboration with physicians, social workers, and other professionals.

Qualificações

• Diploma or Associate Degree in nursing; Bachelor’s Degree preferred.
• Current RN license, active and unrestricted; CPR required.
• Nurse Licensure Compact allows practice across compact states.
• Minimum 2 years related experience in life-limiting illnesses; hospice/palliative care experience preferred.
• Experience with pediatric population for Kids Path Nurse with CPR training.
• Computer proficiency required.

Responsabilidades

• Coordinate patient/caregiver care; conduct nursing assessments.
• Make visits; follow plan of care, evaluate responses, revise as needed.
• Develop and evaluate plans of care; provide education to patient/caregiver.
• Emergency triage by telephone and home visits; communicate with interdisciplinary team.
• Maintain clinical documentation and medication profiles; ensure infection control.
