Job summary

VIA Health Partners is a leading, community-based hospice and palliative care provider. We coordinate patient and family care, conducting nursing assessments and home visits to implement and adjust care plans.

We require an RN with a nursing degree and an active compact license; CPR is a must, CHPN preferred, with at least two years in direct life-limiting illness care. Pediatric experience and computer proficiency are valued as part of our team environment.

Qualifications

• A Diploma or Associates Degree in nursing is required; Bachelor's Degree preferred.
• Current, active, and unrestricted RN license (multistate) required.
• Current CPR certification required; CHPN preferred.
• Minimum 2 years related hospice/palliative care experience.

Responsibilities

• Coordinate patient/caregiver care with an interdisciplinary team.
• Conduct nursing assessments to support patient/caregiver needs.
• Make home visits and provide appropriate interventions.
• Develop, implement, and evaluate plans of care.
• Triage emergencies by telephone and home visits.
• Maintain clinical documentation and ensure compliance with guidelines.
