Job summary

Navian Hawaii is seeking an After Hours Nurse to provide clinical nursing support during non-core hours, including telephone assessments, home visits, and documentation in accordance with hospice standards.

Based in Oahu, the role requires travel to patient homes and facilities; you will collaborate with the Patient Care Manager and Interdisciplinary Team, perform urgent interventions, update physicians, and ensure timely reporting by 7:00 AM.

Qualifications

• Valid RN license in Hawaii.
• Minimum 3 years as staff nurse in an acute hospital or equivalent.

Responsibilities

• Assess patient and family needs by phone and visits, providing skilled nursing interventions.
• Report to attending physician and obtain orders as needed.
• Coordinate with Patient Care Coordinator and interdisciplinary team.
• Document calls, visits, and medication orders per protocol.
• Provide post-call reports to the team by 7:00 AM.
