Job summary

Navian Hawaii invites an experienced Registered Nurse to join our After Hours team. This field-based role requires travel to patient homes and facilities across Oahu, providing skilled nursing support during non-core hours, with documentation and coordination per hospice procedures.

Candidates must hold Hawai'i RN licensure and have 3+ years of acute hospital experience. Expect collaboration with the Patient Care Manager and ongoing communication with physicians and interdisciplinary staff.

Qualifications

  • Valid Registered Nurse License in Hawai‘i.
  • 3 years of staff nurse experience in an acute hospital or comparable setting.
  • Ability to exercise independent clinical judgment and problem-solve in complex patient care scenarios.

Responsibilities

  • Provide telephone assessments, home visits, and documentation per hospice standards.
  • Seek physician consultation and implement orders as directed.
  • Document all calls, visits, and medications in the EMR.
  • Provide clinical reports to the Interdisciplinary Team and coordinate with Patient Care Manager.

Under the supervision of the Patient Care Manager, the After Hours Nurse provides clinical nursing support during non-core business hours. This includes telephone assessments, home visits, and documentation, all in accordance with hospice standards and procedures.

Primary Responsibilities
  • Uses clinical judgment and assesses needs of the patient and their families and initiates intervention to meet the immediate needs by utilizing principles of palliative care and following (but not limited to) the following guidelines:
    • Assesses the need for skilled nursing by phone
    • Makes home visits to calm a situation, to provide necessary skilled interventions, and when a patient expires
    • When indicated, gives pertinent report to attending physician and/or obtains new medication orders
    • Reports any administratively complicated situation to the Patient Care Coordinator
  • Seeks consultation from patient’s primary care physician or Hospice Hawai’i Medical Director and implements orders/recommendations
  • Makes routine/follow up calls or home visits to new admissions and current patients while on duty, utilizing backup system as needed, in accordance with Navian Hawai’i’s 24 hours per day 7 days per week service and admission policy
  • Obtains updated patient information and physicians orders by:
    • Accessing report via the voicemail system or directly from the RN Case Managers
    • Reviewing patient medical records in the EMR system or from another On-Call RN with whom the start of work interfaces, whichever is appropriate to the timing of the start of duty hours
  • Clears calls with answering service within 10 minutes of receiving a phone message
  • Documents all phone calls, home visits, and medication orders in accordance with Hospice Hawai’i protocol
  • Provides clinical report to Interdisciplinary Team regarding all occurrences during after-hours shift and interventions done for those occurrences by 7:00AM
  • If patient calls and/or visits are initiated within the last hour of the shift, call logs must be updated and re-distributed to staff before the end of the work shift
  • Assumes the role of either primary or backup call when other After Hours staff is on leave
  • Other related duties as assigned by the Patient Care Manager or Director of Clinical Operations
Knowledge and Skills Requirements
  • Ability to exercise independent and sound judgment in planning and providing patient care
  • Ability to problem-solve, demonstrate reason and analyze situations
  • Skilled in providing holistic nursing care to patients
  • Ability to identify and provide for psychosocial needs of caregivers
  • Ability to be flexible and organized in planning
  • Ability to establish and maintain effective working relationships with staff, clients, family members, to the public, and representatives of other agencies and organizations
  • Ability to communicate effectively with all levels of individuals
  • Basic computer knowledge of EMRs and MS applications such as Word, Excel, and Outlook
Experience and Education Requirements
  • Valid Registered Nurse License in the State of Hawai’i
  • 3 years of experience as a staff nurse in an acute hospital, or comparable, relevant experience required

  • TB Clearance
  • CPR and BLS Certification
Working Conditions
  • Indoors in-patient homes, nursing, and acute care facilities as well as outdoors in all types of weather
Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours
  • Saturday and Sunday either 8am-4pm, 4pm-12am or 12am-8am

$44.71 - $47.60 per hour
