Job summary

Navian Hawaii is seeking an After Hours Nurse to provide clinical nursing support during non-core business hours, including telephone assessments, home visits, and documentation in alignment with hospice standards.

Under the supervision of the Patient Care Manager, you will collaborate with physicians, perform on-call duties across Oahu, and report to the Interdisciplinary Team while ensuring 24/7 service adherence.

Qualifications

• Valid RN license in Hawai'i is required.
• Minimum 3 years as a staff nurse in an acute hospital or equivalent.
• Background check, drug screen and TB clearance are required.

Responsibilities

• Provide after-hours clinical nursing care by phone and at home.
• Coordinate with physicians and Hospice Hawai'i Medical Director as needed.
• Document calls, visits and medication orders per Hospice Hawai'i protocol.

Under the supervision of the Patient Care Manager, the After Hours Nurse provides clinical nursing support during non-core business hours. This includes telephone assessments, home visits, and documentation, all in accordance with hospice standards and procedures.

Primary Responsibilities

• Uses clinical judgment and assesses needs of the patient and their families and initiates intervention to meet the immediate needs by utilizing principles of palliative care and following (but not limited to) the following guidelines:
  • Assesses the need for skilled nursing by phone
  • Makes home visits to calm a situation, to provide necessary skilled interventions, and when a patient expires
  • When indicated, gives pertinent report to attending physician and/or obtains new medication orders
  • Reports any administratively complicated situation to the Patient Care Coordinator
• Seeks consultation from patient’s primary care physician or Hospice Hawai’i Medical Director and implements orders/recommendations
• Makes routine/follow up calls or home visits to new admissions and current patients while on duty, utilizing backup system as needed, in accordance with Navian Hawai’i’s 24 hours per day 7 days per week service and admission policy
• Obtains updated patient information and physicians orders by:
  • Accessing report via the voicemail system or directly from the RN Case Managers
  • Reviewing patient medical records in the EMR system or from another On-Call RN with whom the start of work interfaces, whichever is appropriate to the timing of the start of duty hours
• Clears calls with answering service within 10 minutes of receiving a phone message
• Documents all phone calls, home visits, and medication orders in accordance with Hospice Hawai’i protocol
• Provides clinical report to Interdisciplinary Team regarding all occurrences during after-hours shift and interventions done for those occurrences by 7:00AM
• If patient calls and/or visits are initiated within the last hour of the shift, call logs must be updated and re-distributed to staff before the end of the work shift
• Assumes the role of either primary or backup call when other After Hours staff is on leave
• Other related duties as assigned by the Patient Care Manager or Director of Clinical Operations

Knowledge and Skills Requirements

• Ability to exercise independent and sound judgment in planning and providing patient care
• Ability to problem-solve, demonstrate reason and analyze situations
• Skilled in providing holistic nursing care to patients
• Ability to identify and provide for psychosocial needs of caregivers
• Ability to be flexible and organized in planning
• Ability to establish and maintain effective working relationships with staff, clients, family members, to the public, and representatives of other agencies and organizations
• Ability to communicate effectively with all levels of individuals
• Basic computer knowledge of EMRs and MS applications such as Word, Excel, and Outlook

Experience and Education Requirements

• Valid Registered Nurse License in the State of Hawai’i
• 3 years of experience as a staff nurse in an acute hospital, or comparable, relevant experience required

Employment Requirements

• A legally compliant background check
• Drug screen & physical exam
• Reliable transportation with valid registration and insurance
• TB Clearance
• CPR and BLS Certification

Working Conditions

• Indoors in-patient homes, nursing, and acute care facilities as well as outdoors in all types of weather

Work Location

This position is based in the field, requiring travel to patient homes, facilities, and offices throughout Oahu using a personal motor vehicle

Hours

• Monday through Friday 4pm-12am OR 12am-8am
• Saturday through Sunday 8am-4:30pm OR 4pm-12am
• Part-Time employees must be available to work a minimum of 3 shifts per week
• Per Diem employees must work a minimum of two shifts per month

$44.71 - $47.60 per hour
