Primary Responsibilities

Patient Care

Implements plan of care developed by Facilities/Case Manager
Conducts individualized focused patient assessments; collecting data based on the patient’s immediate condition or needs
Performs appropriate treatments as ordered by physician in an accurate and timely manner
Provides individualized patient/family education customized to the patient/family
Documents patient assessment findings including physical/psychosocial responses to nursing intervention and progress towards pain/symptom management
Maintains confidentiality in matters related to patient, family and client facility staff
Provides care in a non-judgmental, non-discriminatory manner that is sensitive to the patient/family diversity, preserving their autonomy, dignity and rights
Reports patient condition to appropriate personnel during each shift – i.e. changes in patient condition to case manager, medical director, patient’s physician
Works in conjunction with the Social Worker and Case Manager of the facility to implement a specific plan of care for each patient
Care provided reflects compliance with Federal and State regulations, along with Hospice Hawai'i, Inc. policies and procedures

Knowledge And Skills Requirements

Demonstrates knowledge, attitudes, behaviors and skills that are consistent with the scope of practice for hospice and palliative RNs
Ability to establish and maintain effective working relationships with staff, patient/family members
Ability to communicate effectively and professionally
Ability to handle crisis situations in a professional manner

Ability to maintain records and prepare related reports
