Hiawatha Homes, Inc. seeks an Advocate to develop and maintain communications between individuals supported, families, HH, and community agencies.

You will coordinate admissions and discharges, monitor rights, and assist with family issues or grievances. Key responsibilities include serving as staff representative for Admissions/Discharge Committee, maintaining records, supporting Positive Support Practices, and collaborating with community services such as Apple Tree Dental.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Sociology, Special Education, or related field; 2+ years of experience with individuals with intellectual/physical disabilities or 65+ years old.
• Associate’s degree in related field and 3+ years of experience with individuals with intellectual/physical disabilities or 65+ years old.
• Ability to speak, write, and comprehend English.
• Valid driver’s license; able to pass background check.

Responsibilities

• Develop and maintain communications between individuals supported, families, HH, and community agencies.
• Coordinate admissions and discharges; monitor rights and report abuse/neglect.
• Serve as staff representative for Admissions/Discharge Committee; maintain records.
• Advocate duties: review plans, support positive practices, follow-up on Behavioral Intervention Reports, and suggest internal improvements.
• Collaborate with HH Business Office regarding financial status; complete Representative Payee forms as required.
• Develop and distribute annual survey as required by 245D.
