Job summary

Hiawatha Homes, Inc. is seeking a House Coordinator to manage a designated program, oversee individual support, medical needs, and supervisory duties while maintaining clear communication with the team.

You will ensure plans and records meet licensing standards and support community integration for individuals served. The role involves coordinating intake/discharge, scheduling, staff supervision, and budget oversight.

Qualifications

• Is at least 18 years of age.
• Able to speak, write, and understand English.
• Possesses a valid driver’s license and clean driving record.
• Passes a State of Minnesota background check before working unsupervised.
• Willing and able to work with and support both males and females.

Responsibilities

• Delivers person-centered support with dignity and respect for individuals.
• Collaborates with Program Nurse to manage medical needs and document appropriately.
• Oversees program operations, including intake/discharge and licensing compliance.
• Supervises Direct Support Professionals and ensures training and performance are up to date.
• Maintains effective communication with families, guardians, and team members.
• Ensures program budgets, schedules, and documentation are accurate and timely.
