Responsibilities

• Evaluate, diagnose, and manage patients with acute illnesses and chronic diseases.
• Develop individualized treatment plans including medications, screenings, and preventive services.
• Order and interpret diagnostic tests and coordinate specialty referrals when needed.
• Conduct annual wellness visits and preventive screenings.
• Collaborate with APPs, nurses, pharmacists, and care managers to ensure comprehensive care.
• Document medical decision-making and outcomes in the EHR.
