• Master's-level clinical training within the APP scope.
• Collaborating with physicians to coordinate patient care.

Responsabilités

• Evaluate, diagnose, and manage patients with acute illnesses and chronic diseases within primary care.
• Develop individualized treatment plans including medication management and preventive services.
• Order, perform, and interpret labs, imaging, and other diagnostic tests.
• Perform office-based procedures as appropriate.
• Conduct annual wellness visits and preventive health screenings.
• Coordinate referrals to specialty care and other providers when needed.
• Educate patients and caregivers on diagnoses, medications, and lifestyle changes.
• Document encounters and clinical decision-making in the EHR per policy.
