Sentara Community Care in Norfolk, VA, seeks a seasoned Family Nurse Practitioner to join our on-site team. You will deliver care across family medicine, pediatrics, and adult populations with complex issues, working within a diverse care team including RN, counselors, and care management staff.

1+ year APP experience is required; post-masters training in community health care is advantageous. This role features a collaborative environment with a focus on community health outcomes and education.

Qualifications

• FNP certification preferred; 1+ year APP experience required.
• Experience in community health center or FQHC is beneficial.

Responsibilities

• Treat family medicine, pediatrics, and adult patients with complex issues.
• Co-manage patients with a diversified care team (RN, counselors, care management).
• Review quality and outcome metrics with the healthcare team monthly.
• Collaborate with local civic and faith-based leaders on community education programs.

The base pay range for full-time employment is $100,900.80 - $312,000.00 annually.Additional compensation may be available for this role, such as shift differentials, standby/on-call pay, overtime, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
