Endeavor Health is seeking an Advanced Practice Provider (APN/PA) for Primary Care – Family Medicine in Arlington Heights, IL. The role involves working under collaborating physicians, performing exams, ordering tests, and managing patient care across in-clinic and hospital settings.

You will contribute to evidence-based practice, health promotion, and patient education. Qualified candidates hold IL APN/PA licensure, DEA and controlled substances credentials, and have at least 2 years’ APN/PA

Responsabilidades

• Obtain history, perform exams, order imaging and labs, interpret results.
• Maintain records and progress notes; manage treatments.
• Prescribe therapies per protocol and promote wellness.
• Collaborate with health care team to improve patient outcomes.

Arlington Heights, ILFull Time/Part Time: Full Time, 40 hour weekHours: Monday - Friday, between the hours of 7:00am - 5:00pmRotating Saturday's, 8:00am - 12:00pmRotating Call - Approx. one week/yearJob DescriptionExciting opportunity to join Endeavor Health in the Primary Care Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization.
