Endeavor Health in Arlington Heights, IL is seeking an Advanced Practice Provider (APN/PA) for Primary Care/Family Medicine. The role offers full-time hours, Mon-Fri 7:00am-5:00pm with rotating Saturdays.

Under the supervision of collaborating physicians, you will perform exams, order imaging and labs, prescribe therapies, maintain records, and work with a multidisciplinary team to deliver high-quality patient care. IL licensure and DEA required.

Qualifications

• Licensed APN or PA in IL with Federal DEA and Illinois Controlled Substance License
• Master's level program in specialty area
• BLS/CPR certification required
• Minimum of 2 years APN/PA experience in Primary Care

Responsibilities

• Provide primary care under physician supervision
• Take detailed history and perform physical exams
• Order imaging, labs, and other diagnostics and interpret results
• Prescribe therapies and provide patient education
• Maintain records and progress notes; follow patients across clinic and hospital settings
• Collaborate with health care team to achieve optimal outcomes

• Full Time/Part Time: Full Time, 40 hour week
• Hours: Monday - Friday, between the hours of 7:00am - 5:00pm
• Rotating Saturday's, 8:00am - 12:00pm
• Rotating Call - Approx. one week/year

Job Description

Exciting opportunity to join Endeavor Health in the Primary Care Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes.Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

Qualifications:

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Minimum of 2 years APN/PA experience in Primary Care setting required

• Experience in acute, chronic disease, and preventative care
• Excellent communication skills and empathetic/compassionate about patient care
