Endeavor Health in Arlington Heights, IL seeks an Advanced Practice Provider (APN/PA) for our Primary Care – Family Medicine team. Under the direction of collaborating physicians, you will take detailed histories, perform physical exams, order imaging and labs, interpret results, and guide treatment plans.

Maintain records, educate patients, and collaborate with a multidisciplinary team to promote wellness and high-quality care in both clinic and hospital settings.

Qualifications

• APN/PA licensure in IL (or eligibility) required.
• DEA license and Illinois Controlled Substance License required.
• Minimum of 2 years APN/PA experience in primary care preferred.
• Master’s degree required; BLS/CPR certification recommended.

Responsibilities

• Provide primary care under physician supervision, including history, exams, orders, and interpretation of results.
• Instruct patients on therapies, post-procedure care, and preventive health.
• Maintain accurate medical records and progress notes.
• Collaborate with physicians and care teams to achieve optimal outcomes.
• Promote wellness and manage acute and chronic illnesses within clinic and hospital settings.

Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization.
