Job summary

Endeavor Health in Chicago, IL seeks an Advanced Practice Provider (APN/PA) in Family Medicine to join our outpatient team. You will collaborate with physicians to deliver comprehensive primary care, perform histories and physicals, order studies, and manage ongoing patient care.

We offer a full-time schedule (40 hours/week) with rotating early/late shifts and weekend call, a supportive environment, and opportunities for professional growth within a large integrated health system.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA license required
• Education: Completion of master's level program in specialty area
• Certification: BLS/CPR certification required

Responsibilities

• Provide care to patients in the outpatient Family Medicine setting.
• Order imaging, lab tests, and diagnostic procedures and interpret results.
• Instruct patients on therapies and pre/post operative instructions; maintain records and progress notes.
• Collaborate with physicians and health care team to achieve optimal patient outcomes.

Job Description

Exciting opportunity to join Endeavor Health in Primary Care - Family Medicine Department.Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes.Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

• Education: Completion of master's level program in specialty area


• Certification: BLS/CPR certification required


• Experience: Prior APN/PA experience preferred
