Job summary

Crossroads Health in Mentor, OH is seeking an Advanced Practice Provider (PA/NP/APRN) to deliver comprehensive primary care within an integrated health team. The APP works with physicians and a multidisciplinary team to provide patient-centered care across the life span.

The role supports behavioral health integration, care coordination, and medical management in accordance with Ohio regulations and trauma-informed practices. Ohio licensure and Medicare CDS credentials are important.

Qualifications

• Master’s Degree required and Ohio licensure as PA/NP/APRN.
• Experience in hospital or family practice settings as primary care APP preferred.
• Licensure to practice in Ohio; Medicare provider number and CDS registration desirable.

Come and be a part of our Primary Care Health Center with locations in Mentor, Willoughby and Euclid. The Advanced Practice Provider (APP) is a Physician Assistant (PA), Nurse Practitioner (NP), or Advance Practice Registered Nurse (APRN). The APP works in accordance with a standard collaborative care arrangement with a physician or to provide primary care services collaboratively within a multidisciplinary health team. The APP is responsible for providing comprehensive patient-centered care in an integrated healthcare setting. This position is responsible for individual services to patients assigned by Crossroads Health in the Integrated Healthcare Center

Additionally, the Prescriber will:

• Provide general comprehensive primary medical care services to individual patients across the life span in the office setting including behavioral health assessment, medication evaluation and medication management, and prescription of medication in compliance with Ohio state regulations using a trauma informed approach.
• Provide diagnosis, screening, education, treatment, referrals, general medical procedures, and follow-up of services.
• Provide medical coordination of ongoing treatment involved with the transfer or discharge planning of health center patients in various settings.

Position Requirements:

• Master’s Degree (minimum)
• Licensed in the State of Ohio as a Physician Assistant, Nurse Practitioner, Advanced Practice Registered Nurse
• Experience in a hospital or family practice setting as a primary care APP (one year preferred)
• Medicare Provider Number
• Controlled Substance Registration License in Ohio
• Professional Liability Insurance. (employer provided)
• Standard Care Arrangement (annually) or Supervision Agreement (whichever applicable) (employer provided)
• BLS/First Aid (employer provided)
