Job summary

Qualifications

• Master’s Degree required and Ohio licensure as PA/NP/APRN.
• Experience in hospital or family practice settings as primary care APP preferred.
• Licensure to practice in Ohio; Medicare provider number and CDS registration desirable.

Responsibilities

• Provide comprehensive primary medical care and behavioral health integration within an integrated health team.
• Coordinate ongoing treatment and discharge planning across settings.
