Job summary

Cleveland Clinic Health System is seeking a qualified Advanced Practice Provider (NP/PA/CNS) for the Palliative Medicine Program at Mercy Hospital in Canton. The role focuses on new consults, follow-ups, symptom management, and goals of care in a high-acuity setting.

Inpatient clinic-based work with occasional weekend/holiday coverage; ACHPN certification must be obtained within two years of hire; new graduates with extensive oncology/ICU/hospice experience may be considered.

Qualifications

• MSN required upon hire or within 3 years for external hires and internal hires moving from a non-APRN licensed role to an APRN licensed role.
• Graduate of an accredited APRN program.
• PA: Master's degree in Physician Assistant Studies and/or higher degree from an accredited program.

Responsibilities

• Conducts new consults and follow-ups, and rounds with attending physicians and other team members.
• Manages adult patients in the division, including symptom assessment and management, serious illness conversations, and goals of care.
• Prescribes and monitors medications as allowed by license and policy.
• Educates patients about their disease, treatments, and potential side effects.
• Participates in multidisciplinary rounds and coordinates care with nursing and other teams.
