Endeavor Health in Evanston is seeking an Advanced Practice Provider (APN/PA) for our Pulmonology team to care for new and established patients, plus hospital follow-ups in general pulmonology.

The role is outpatient, full-time, 40 hours weekly, with 36 direct patient care hours and 4 administrative hours, Monday to Friday 8:30 am–5:00 pm, no weekends or call.

Qualifications

• Active IL APN or PA licensure is required.
• Master’s degree in a specialty area.
• BLS/CPR certification required (DEA license preferred).

Responsibilities

• Obtain histories and perform physical examinations.
• Order imaging, labs, and diagnostics; interpret results.
• Prescribe pharmacologic and non-pharmacologic therapies.
• Collaborate with physicians and health care team to optimize outcomes.
• Provide comprehensive health assessments and differential diagnoses.
• Promote wellness and preventive care across settings.

Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed histories and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, formulate differential diagnoses, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevention of illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as a resource and consultant. Positively contributes to safety outcomes and promotes a high-quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization.
