Job summary

Endeavor Health in Chicago seeks an Advanced Practice Provider (APN/PA) to join the Hospital Medicine Department at Swedish Hospital. This full-time role offers day-one responsibilities in a fast-paced hospital setting, with shifts from 12pm–12am and some 7pm–7am.

Travel to Evanston is required as a secondary location. The APN/PA will perform histories, exams, orders, and coordinate care with physicians, aiming to deliver high-quality outcomes while following protocols and ensuring patient

Qualifications

• APN or PA licensure in IL required
• Master’s level program completion
• DEA/Controlled Substance license required

Responsibilities

• Provide care to patients in Hospital Medicine as part of the team
• Obtain detailed history and perform physical examinations
• Order imaging, labs and diagnostic procedures and interpret results
• Maintain records and write progress notes on status, treatments and outcomes
• Collaborate with physicians and health-care team to achieve optimal patient outcomes
• Prescribe pharmacologic and non-pharmacologic therapies under protocol as appropriate

Hourly Pay Range: $57.76 - $80.86 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Hourly Pay Range: $57.76 - $80.86 USD Hourly - As a valued member of an integrated team, your placement within the pay range is dependent on a variety of factors, including but not limited to FTE, prior experience, certifications, degrees, subspecialties, projected volumes, and quality performance.

Job Description

Exciting opportunity to join Endeavor Health in the Hospital Medicine Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures.

Qualifications

License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required

Education: Completion of master’s level program in specialty area

Certification: BLS/CPR and ACLS certification required

Experience: Prior Hospitalist APN/PA experience highly preferred
