The successful candidate will hold IL APN/PA licensure, DEA and CS licenses, with a Master’s degree in a related field and BLS/CPR certification.

Qualifications

• License: Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL required; Federal DEA and Illinois Controlled Substance License required
• Education: Completion of master’s level program in specialty area
• Certification: BLS/CPR certification required
• Experience: Prior APN/PA experience preferred
• Experience working in an outpatient or community health setting

Responsibilities

• Provide care to patients in the Internal Medicine outpatient setting under supervision.
• Obtain detailed history and perform physical examinations.
• Order imaging, labs and diagnostic procedures and interpret results.
• Instruct patients on therapies and post-procedure instructions; maintain records.
• Collaborate with physicians and health care team to achieve optimal outcomes.
• Prescribe therapies as appropriate and participate in standards of practice development.

Full Time: (average of 40 hours/wk.)

Hours: Hours are variable and flexible, based on department needs and candidate availability.

Physicians and APPs have an on-call rotation during the week/weekend/Holiday call.

Outpatient Job Description: Exciting opportunity to join Endeavor Health in the Internal Medicine Department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease. Promotes wellness and prevent illness and injury in various settings for individuals, families, and communities. Works autonomously and in interdisciplinary teams as resources and consultants. Positively contributes to safety outcomes and promoting high quality patient experience. Participates in the development, implementation, and evaluation of service line goals. Supports the strategic plan of the organization. Adheres to all Endeavor Health policies and procedures. APP will see new pts, established pts, walk ins, etc.) If APP has specific procedures that might be done the office will work to ensure supplies and ability to do so are completed.
