Job summary

Community Health Network in Indianapolis is seeking an Advanced Practice Provider to join the Heart Failure team at CHVH. This full-time role is based at our north campus and offers day shifts with weekend and call rotations as determined by the practice.

The position requires a strong inpatient cardiac or ICU RN background, Indiana licensure and certification aligned with advanced practice standards. You will work in a multidisciplinary environment focused on delivering high-quality patient

This Advanced Practice Provider opportunity is located at our Community Heart and Vascular Hospital on our north campus.

• Weekend/call rotation is required. Frequency of each will be determined by the practice.
• Multidisciplinary approach with a team of specialty physicians and advanced practice providers
