Job summary

Hackensack Meridian Health is seeking a Per Diem Hospital at Home Nurse Practitioner/ APN to deliver patient-centered care within a coordinated care model. The APN collaborates with the practice/hospital team to assess and implement care for patients with health and safety needs.

The role emphasizes leadership, direct clinical practice, consultation, coaching, and participation in organizational initiatives, with weekend shifts and holidays required; no on-call.

Qualifications

• Graduate of an NLN/AACN accredited nursing program.
• Master's or Doctorate degree from an accredited Nurse Practitioner program.
• Prescriptive authority through CDS and DEA registration according to New Jersey state law.
• Excellent written and verbal communication skills.
• Proficient computer skills including Microsoft Office/Google Suite.

Responsibilities

• Performs and documents accurate history and physical assessments appropriate for patient age and clinical problem.
• Orders, performs, and interprets laboratory tests and diagnostic studies in collaboration with physicians.
• Manages general medical and surgical conditions with knowledge of pathophysiology and pharmacology advantages/contraindications.
• Prescribes medications safely in accordance with NJ regulations and provides patient education.
• Promotes APN practice recognition through conferences, publications, and presentations.
• Provides patient- and family-centered care and adheres to scope of practice and hospital policies.
