Job summary

Oak Street Health is seeking a Nurse Practitioner/Advanced Practice Provider to deliver high-quality primary care to adults on Medicare in underserved Rhode Island communities. You will conduct comprehensive assessments, manage chronic conditions, and provide preventive care within a patient-centered, team-based environment.

You will educate patients and families, support care plans, and coordinate with physicians, nurses, and social workers.

Qualifications

• Must be a licensed Nurse Practitioner/Advanced Practice Provider.
• Experience delivering primary care to adults on Medicare is a plus.
• Ability to educate patients and families and coordinate with care teams.
