Job summary

Oak Street Health is seeking a Nurse Practitioner to deliver effective, equitable value-based primary care to adults on Medicare, helping them stay healthy and out of the hospital.

The NP will see patients independently and collaborate with physicians within a outpatient collaborative care team, focusing on care coordination, wellness visits, and population health initiatives. Home visits may be performed as needed in a full-time role.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
• National certification in at least one NP specialty (Family NP, Adult-Gerontology, Adult NP, or Gerontological NP).
• Active, non-probationary NP license in the US and a valid DEA license.
• US work authorization is required.

Responsibilities

• Provide exceptional primary care within a value-based model.
• Conduct office visits for routine and acute issues.
• Administer annual wellness visits and health risk assessments with thorough documentation.
• Coordinate care with physicians, specialists, facilities and agencies.
• Lead population health efforts with the care team to ensure appropriate screenings.
• Assist with phone triage and outreach as part of the care team.
• Educate patients about conditions, plans and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties as assigned.
