Job summary

Oak Street Health in Phoenix seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare within outpatient clinics. You will work with a collaborative care team to provide longitudinal care, establish trust, and help patients understand their care plans.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP certificate program graduate.
• Active, non-probationary NP license in current state.
• Active DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Administer Annual wellness visits and HRAs with holistic documentation.
• Care coordination with other providers, facilities, and agencies.
• Population health leadership with the care team.
• Assist care team with phone triage and outreach.
• Educate patients on health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.
• Other duties, as assigned.
