Job summary

Oak Street Health is seeking a Nurse Practitioner, Advanced Practice Provider to deliver effective, equity-focused primary care to adults on Medicare. You will practice in outpatient settings on a collaborative care team with physicians, PAs, MAs, and care managers to keep patients healthy and out of the hospital.

The role emphasizes longitudinal care, care coordination, and population health leadership within a growing network of clinics.

Qualifications

• MSN, DNP, or NP certificate program completed.
• National certification in Family NP, AGPCNP, ANP or Gerontological NP.
• Active NP license and DEA license.
• US work authorization.

Responsibilities

• Provision of exceptional primary care.
• Conduct office visits for routine and acute issues.
• Annual wellness visits and health risk assessments with thorough documentation.
• Care coordination with other providers, specialists, testing facilities, and agencies.
• Population health leadership in coordination with the Care Team.
• Assisting the care team with phone triage and outreach.
• Educating patients on their health conditions, care plans, and treatments.
• Participating in Oak Street Health promotional activities.
• Conducting home visits as needed.
• Other duties, as assigned.
