Qualifications

• Basic Cardiac Life Support (BCLS) certification.

Responsibilities

• Develops and implements collaborative practice agreement and reviews it periodically.
• Assesses health status by history and physical examination.
• Initiates procedures, prescribes/modifies meds per protocol and collaborates with physicians.
• Provides teaching and counseling on health promotion and disease management.
• Documents findings and plan of care in the medical record.
• Consults with physician as established by the agreement.
• Initiates referrals to specialty services and supports continuity of care.
• Participates in team meetings to enhance interdisciplinary care.
• Performs patient record review with collaborating physician at least quarterly.
