Job summary

Qualifications

• Two years of NP experience preferred.
• Neurology experience required.
• New York State License with NP certification.
• Collaborative practice agreement with a physician and approved protocols.
• Epic experience with electronic health records.

Responsibilities

• Develops and reviews clinical practice agreements under protocols.
• Assesses patient health status via history and physical exams.
• Initiates procedures, prescribes and adjusts therapies per protocols.
• Provides health promotion and disease prevention education.
• Documents findings and plans in medical records.
• Consults with physicians as per agreement.
• Initiates referrals to specialty services to ensure continuity.
• Participates in interdisciplinary team meetings.
• Performs regular chart reviews with collaborating physician every 3 months.
