Oak Street Health in Chicago is seeking a Nurse Practitioner to deliver effective, equitable value-based primary care to adults on Medicare, keeping them happy, healthy, and out of the hospital. NPs work independently and collaborate with physicians on a care team.

The role emphasizes longitudinal care, patient relationships, health promotion, and coordination with specialists within a clinic-based team. The position offers full-time hours and comprehensive benefits.

Qualifications

Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
National certification in one of: Family Nurse Practitioner; Adult-Gerontology Primary Care Nurse Practitioner; Adult Nurse Practitioner; Gerontological Nurse Practitioner.
Active, non-probationary state Nurse Practitioner license.
Active DEA license.
US Work Authorization.
Experience in primary care: internal medicine, geriatrics, or family medicine.
Understands HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
Passion for teamwork and collaboration across functions.
Desires to be part of an innovative population health model.
Bilingual proficiency in applicable areas.

Responsibilities

Provision of exceptional primary care.
Conduct office visits for routine and acute issues.
Administer Annual wellness visits and health risk assessments, which require a holistic view of health and a focus on thoughtful, accurate, and specific documentation.
Care coordination with other providers, specialists, testing facilities, and agencies.
Population health leadership, in coordination with the Care Team (e.g., making sure all eligible females get their evidence-based breast cancer screening every 2 years).
Assisting the care team with phone triage and outreach.
Educating patients on their health conditions, care plans, and treatments.
Participating in Oak Street Health promotional activities.
Conducting home visits as needed.
Other duties, as assigned.
