In this Optum Care Delivery role you’ll coordinate care for a defined panel, perform exams and treatments, and work with a team to optimize patient outcomes in a primary care setting.

Qualifications

• Graduate of an accredited NP program with masters preferred.
• Licensed by MA Board of Nursing.
• National board certification required.
• Mass DPH controlled substances registration or ability to obtain post-hire.
• DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification and appropriate professional certifications.

Responsibilities

• Comprehensive clinical and administrative coordination for a panel of patients.
• Examination, diagnosis and treatment in a primary care setting, with care plan supervision.
• Oversees monitoring/management of acute and chronic illnesses; adjusts regimens as needed.
• Partners with patients and families to manage care plans.
• Provides consultation regarding hospitalized patients in the panel.
• Directs and supports staff to ensure departmental effectiveness.

Education

Graduate of an Accredited Nurse Practitioner Program (Masters level preferred)

Licensed by the Commonwealth of Massachusetts board of registration in Nursing

National Board Certification

Mass DPH Controlled Substances Registration

DEA licensure/prescriptive authority

Maintains BLS certification
