Qualifications

• Graduate of an Accredited Nurse Practitioner or Physician Assistant Program.
• Active, unrestricted Massachusetts NP or PA license.
• National Board Certification.
• Current active Mass DPH CS Registration or ability to obtain post-hire.
• Current active DEA licensure/prescriptive authority or ability to obtain post-hire.
• Maintains BLS certification, professional certification as appropriate.

Responsibilities

• Comprehensive, longitudinal clinical and administrative coordination of patient care.
• Examination, diagnosis and treatment in primary care; manage care plan with referrals.
• Monitor and manage acute and chronic illnesses; supervise care plan.
• Partner with patients and families to manage the care plan.
• Provide consultation for hospitalized patients in the panel.
• Direct and support staff to ensure department effectiveness and efficiency.
