Qualifications
- Two (2) years of registered nurse experience.
- Minimum two (2) years of experience as a Nephrology Nurse with dialysis experience; in peritoneal dialysis, home hemodialysis, or a combination of both.
- Graduation from an accredited nursing school or equivalent; bachelor's degree preferred.
- Current state RN licensure.
- Current Healthcare Provider certification (CPR) required.
- CNN/CDN Certification or within 12 months.
Responsibilities
- The Home Therapies Dialysis Nurse, Advanced will ENGAGE, ADVOCATE, and ROLE MODEL as an SHC coordinator of patient care, collaborating with other care providers and health team members to provide required care.
- Will perform home visits to determine that the home environment is suitable for patient dialyzing at home and whether or not the home will be a safe and appropriate setting in compliance with CMS regulations.
