Satellite Healthcare seeks an experienced Home Therapies Dialysis Nurse, Advanced to lead patient-centered care coordination and education. You will collaborate with a multidisciplinary team to ensure safe, ethical, and effective home dialysis delivery, promoting patient independence and quality outcomes.

You will participate in continuing education, quality improvement, and apply research findings, while visiting homes to assess environments for safety and CMS compliance.

Qualifications

• Two years of registered nurse experience.
• Minimum two years in nephrology dialysis with dialysis experience.
• Bachelor's degree in nursing preferred.
• Current state RN license and CPR certification.

Responsibilities

• Engage, advocate and role model as SHC coordinator of patient care with the care team.
• Educate patients, support informed consent, and promote independence.
• Engage in continuing education, QA, and research application.
• Perform home visits to assess home suitability for home dialysis in CMS compliance.
