HarmonyCares presents a Geriatric Advance Practice Provider opportunity in Denton, TX. This salaried position focuses on in-home primary care for high-acuity patients within a value-based model, with no weekends or evenings and a strong annual quality-based bonus.

The APP will manage patient panels across Denton and nearby areas, supported by an assistant in select markets and a company car where available. Competitive benefits include 401K with match and CME/license stipends.

Qualifications

• Licensed NP or PA in the state where HarmonyCares Medical Group operates.
• Federal DEA license or eligibility required.
• Geriatric or home health experience preferred.

Responsibilities

• Provide in-home primary care to high-acuity patients in Denton area Monday–Friday.
• Deliver patient-centered care with focus on quality and outcomes; avoid weekends/evenings.
• Coordinate with the care team to support patients and families.

Patient panel across Denton, Aubrey, Lewisville, Carrollton areas with 30-45 min patient visits seeing approx. 6-9 patients per day.

The Advance Practice Providers we seek are those who have directly related experience in home health, geriatrics, and primary care. The APPs who wants to practice medicine with a focus on patient care, not volume. We want our clinicians to take the time needed to truly address the patient's needs.

Qualifications

• Licensed as a NP or PA in the State where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Must maintain a valid driver's license and good driving record
