Job summary

MyCare Medical is seeking a full-time, certified Nurse Practitioner/Physician Assistant to be the primary clinical provider for MyCare patients in Pembroke Pines, FL. The practice specializes in primary care with a physician-founded, dyad-led structure that supports providers to focus on patient care.

Responsibilities include a Monday–Friday 8am–5pm schedule with on-call rotation as needed. The team uses eClinical Works and emphasizes continuity of care, preventive health, and chronic disease

Qualifications

• Must hold current licensure as an Acute, Adult/Gerontological or Family Nurse Practitioner or physician assistant in state of practice
• DEA and CDS licensure (if required in state)
• CPR/BLS certification up to date
• Minimum 2 years NP experience preferred
• Certification through AANP/ANCC or PA certification focusing on adult medicine

Responsibilities

• Provide outpatient services Monday–Friday 8am–5pm and participate in on-call schedule as needed
• Utilize eClinical Works (eCW) and partner programs for patient care
• Deliver continuity of care, health maintenance, and disease prevention with patients and families
• Work in an integrated team-based setting to improve patient outcomes
• Prioritize preventative care and chronic disease management to reduce costs
• Care for an adult patient population with complex comorbidities
• Support in ensuring efficient clinic operations and patient care workflows

We’ve created a structural support system to ensure that our providers can focus solely on patient care.

Responsibilities

• Monday – Friday, 8am-5pm
• Rotating and flexible on-call schedule.
• We use eClinical Works (eCW), along with partnering programs to ensure ease of use for our patients and practices.
• We focus on continuity of care, health maintenance, and disease prevention, working alongside patients and their families to prevent, diagnose, treat and manage a population with multiple comorbidities.
• Integrated team-based care: Working as a multidisciplinary team to improve patient outcomes by meeting their needs and preferences.
• Prioritizing preventative care and proactive management of chronic conditions to improve patient outcomes, decrease patient’s costs, and increase revenue for the practice.
• Providers must be comfortable managing a primarily adult-aged patient population.

Qualifications

• Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC)
• Certification through the American Academy of Nurse Practitioners (AANP) OR American Nurses Credentialing Center (ANCC) program, focusing on adult medicine
• Certification through an accredited PA certification program focusing on adult medicine
• Currently licensed as an Acute, Adult/Gerontological or Family Nurse Practitioner in the state of practice or the ability to obtain a license prior to employment
• Ideal Candidate, with a minimum of two years of experience as Nurse Practitioner
• Holds or is eligible for DEA licensure and CDS licensure (if required in the state of practice)
• Up-to-date CPR Certification and Basic Cardiac Life Support certification

Required Skills

• Strong organizational and prioritization skills
• Ability to use Electronic Medical Record (EMR) system
• 2 years of experience of Medical Advantage, HEIDS, MRA and ICD-10 Coding
• Comfortable managing a primarily adult-aged patient population
• Demonstrated ability to problem-solve complex and multifaceted situations
