Job summary

Palm Medical Centers is seeking a Primary Care Nurse Practitioner to join our premier team. This in-person, value-based care role focuses on keeping patients healthy with autonomy and work-life balance.

Job Requirements:

• Have at least 2-3 years’ experience in the primary care/value-based care setting, working with Medicare Advantage, Medicare, and Medicaid patient populations
• Must have an active and unrestrictive Florida APRN license
• Must have DEA license or eligible to obtain it within 2 months of date of hire
• Strong understanding of chronic disease management, preventative care, and quality metrics in a value-based care model
• Excellent documentation and patient relationship-building skills
• Proficient with EMR systems (eCW a plus!)
