Job summary

Ascend Hospice & Palliative Care PA is hiring Nurse Practitioners for per diem, with a flexible schedule in the Pennsylvania hospice program.

The NP will conduct visits, share findings with the interdisciplinary team and help coordinate care models, while supporting quality palliative care standards. A Master’s in Nursing and PA NP license are required, with 2–4 years hospice experience preferred and travel willingness.

Qualifications

• Must have a Nurse Practitioner's license in PA.
• 2-4 years experience working within hospice or related field.
• Palliative care experience preferred.
• Must have a Master's Degree.
• Must have a valid driver's license and a reliable source of transportation.
• Must be willing to travel.

Responsibilities

• Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
• Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
• Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
• Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
• Prepare and maintain accurate patient records, charts and documents.
• Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
• Ensure collaboration with patients, caregivers, medical directors and members of IDT.
