Job summary

AccentCare, Inc. is seeking a part-time Nurse Practitioner in Palliative Care for Loveland and surrounding areas. You’ll evaluate patients, develop care plans and collaborate with a multidisciplinary team to deliver compassionate care during chronic and terminal illness.

Qualifications include NP licensure in the state, DEA license, CPR certification, and ability to travel between locations. Compensation is $50.00-$62.00 per hour with coverage during standard daytime hours.

Qualifications

  • Licensed NP with approved nursing education.
  • DEA license in practice state preferred.
  • CPR certification required or preferred.

Responsibilities

  • Develop and implement care plans for patients with chronic/terminal illness.
  • Conduct patient assessments, histories, and physicals with symptom management.
  • Monitor progress and update medical leadership; coordinate discharge planning and education.
  • Communicate with supervising and attending physicians regularly to review care.
  • Coordinate with other care team members and community resources to optimize patient care.
  • May provide support at time of death.
  • Prescribe medications as permitted by state regulations.
  • Participate in continuum of care for hospice-appropriate patients.

Be the Best Palliative Care Nurse Practitioner You Can Be

If you meet these qualifications, we would love to meet you:

  • Graduated from an approved school of professional nursing
  • One year experience as a Nurse Practitioner is preferred
  • Licensed to practice as a Nurse Practitioner in the state of agency operation
  • Active DEA license in the state where care is being provided.
  • Possess and maintain valid CPR certification while employed in a clinical role, preferred
  • Must be a licensed driver who can travel to all business locations
  • Meet the regulations and requirements of the state(s) in which program provides services

Responsibilities:

As a Palliative Care Nurse Practitioner, you will:

  • Develop and implement clinical plans of care for patients with chronic and terminal illnesses
  • Perform patient assessment, history and physical; recommendations on aggressive symptom-management
  • Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching
  • Maintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis
  • Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family
  • May provide support at time of death
  • Prescribe medication as permitted by the state and/or the State Board of Nursing
  • Participate in continum of care for patients that are hospice appropriate and ongoing education programs.
