Job summary

Ascend Hospice & Palliative Care PA is actively seeking a Registered Nurse for on-site weekend PRN coverage in North Wales, PA. The RN will assist with on-call, case management, and visits to support continuous, coordinated hospice care for patients and families.

The position requires an Associate's Degree in Nursing, an active state RN license, and at least 2 years of nursing experience (hospice preferred). Must have a valid driver's license and be willing to travel.

Responsibilities

• Perform initial patient and family assessments to determine hospice care needs
• Regularly reassess nursing needs and adjust care plans
• Utilize assessment data to determine care plan requirements
• Coordinate with healthcare team to ensure continuous, well-coordinated care
• Counsel patients and families on end-of-life needs
• Initiate appropriate nursing procedures focused on palliative care
• Communicate with physicians about patient needs and condition changes
• Supervise Hospice Aides as assigned

Now Hiring – Registered Nurse – North Wales, PA

Hours: PRN Weekends

TheRegistered Nursewill be responsible for assisting the Team with On Call/Case Management/Visits to aid in coverage and call outs. Job duties can include, but are not limited to:

1. Completing an initial assessment of patient and family to determine hospice care needs
2. Regularly re-evaluates the patients nursing needs
3. Use assessment data to determine care plan needs
4. Collaborate with the entire healthcare team, patient and family, to help direct the work of the team to provide are that is continuous and well-coordinated
5. Counsels the patient and family in meeting end of life needs
6. Initiates appropriate preventative and palliative nursing procedures
7. Communicates with the physician regarding patient's needs and reports any changes in the patient's condition
8. Supervises the Hospice Aides

Education:

Associate's Degree in Nursing

Professional Experience:

• Must have a State Certified RN license
• 2 years' experience as a professional nurse, hospice experience preferred
• Must have a valid Driver's license and reliable transportation
• Must be willing to travel
• Strong verbal and written communication skills.
