Job summary

AccentCare is seeking a full-time Nurse Practitioner (NP) in Palliative Care in Florida for 0.8 FTE with a salary range of $90,000 to $98,000. You will join a purpose-driven team focused on compassionate, high-quality care.

The role involves developing clinical care plans for patients with chronic and terminal illnesses, performing comprehensive assessments, and coordinating with physicians and care teams to ensure optimal symptom management and seamless discharge planning.

Qualifications

• Graduated from an approved school of professional nursing.
• One year experience as a Nurse Practitioner is preferred.
• Licensed to practice as a Nurse Practitioner in the state of agency operation.
• Active DEA license in the state where care is being provided.
• Possess CPR certification while employed in a clinical role, preferred
• Must be a licensed driver who can travel to all business locations
• Meet the regulations and requirements of the state(s) in which program provides services

Responsibilities

• Develop and implement clinical plans of care for patients with chronic and terminal illnesses
• Perform patient assessment, history and physical; recommendations on aggressive symptom-management
• Monitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge planning; and patient/family teaching
• Maintain communication with the supervisory physician(s) and attending physician to review patient records and care plans on a routine basis
• Initiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family
• May provide support at time of death
• Prescribe medication as permitted by the state and/or the State Board of Nursing
• Participate in continuum of care for patients that are hospice appropriate and ongoing education programs.

If you meet these qualifications, we would love to meet you:
