Job summary

Ascend Hospice & Palliative Care PA is seeking Nurse Practitioners on a per diem basis in Pennsylvania. You will conduct visits to patients in palliative care, communicate findings to the interdisciplinary team, and help access appropriate care models.

This role includes travel and mileage reimbursement and offers competitive salaries with comprehensive benefits. Requirements include PA NP license, 2–4 years hospice experience, a Master's in Nursing, and a valid driver's license.

Qualifications

Must have a Nurse Practitioner's license in PA.
2-4 years experience in hospice or related field.
Palliative care experience preferred.
Must have a Master's Degree.
Must have a valid driver's license and reliable transportation.
Must be willing to travel.

Responsibilities

Conduct visits to patients on palliative care in accordance with local, state and federal regulations.
Communicate visit findings to designated members of the interdisciplinary team, including the National Director of Palliative Care.
Facilitate access to appropriate support models of care (i.e., hospice, rehabilitation, on-going aggressive treatment).
Consult with other members of interdisciplinary team to contribute to the patient's plan of care.
Prepare and maintain accurate patient records, charts and documents.
Assist in development of clinical practice guidelines/standards in support of quality palliative care as needed.
Ensure collaboration with patients, caregivers, medical directors and members of IDT.
