Qualifications

• Minimum 3 years of relevant experience required.
• Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire.
• Must hold a current RN license in North Carolina for BSN-prepared candidates.

Responsibilities

• Manage assigned caseload for timely assessment and care transitions.
• Coordinate services such as Home Health and outpatient follow-up.
• Develop and evaluate individualized discharge plans.
