Job summary

Duke University Hospital in Durham, NC, seeks a Case Manager for Adult Transplant with LCSW license or LCSW-A. The role focuses on psychosocial assessments, discharge planning, and care coordination across the continuum of care.

A Master’s in social work and 3 years’ experience are preferred, with CCM/ACM certification within 3 years of hire. On-site position with relocation and commitment bonuses available.

Qualifications

• Master's degree in social work preferred for Complex and Transplant CM positions.
• 3 years of relevant experience.
• Requires Case Management Certification (ACM or CCM) within 3 years of hire; holds or to obtain LCSW/ LCSW-A as applicable.

Responsibilities

• Provide psychosocial assessments of patients and families to identify emotional, social, and environmental needs.
• Develop and implement comprehensive psychosocial treatment plans including individual, family, and group therapies as appropriate.
• Coordinate discharge planning and care transitions across the continuum of care; liaise with team and community resources.
• Maintain timely documentation of assessments, plans, and actions per departmental guidelines.
• Engage in Quality Assurance/Performance Improvement activities as requested.
