Job summary

The Hope Adult Center in Mars, PA is seeking an Adult Services Coordinator to support the implementation of adult services and uphold the center’s integrity and efficacy.

The role requires a bachelor’s degree (master’s preferred), at least 1 year of experience with adults with special needs, and experience supervising multidisciplinary teams. Knowledge of 55 Pa Code §6100, PROMISe, and HCSIS is a plus.

Qualifications

• Bachelor’s degree required; Master’s degree preferred.
• At least 1 year of working with adults with special needs.
• Experience supervising multidisciplinary teams and managing QA initiatives.
• Strong knowledge of PA regulations, waiver services, and community integration.

Responsibilities

• Ensure services comply with professional standards and regulatory requirements.
• Maintain documentation per local, state, and federal requirements.
• Communicate professionally with clients' representatives or guardians.
• Implement engaging programs to support client satisfaction and development.
• Assist with ISP writing to ensure legal compliance and adherence to ISPs.
• Maintain adherence to individual support plans and participate in transition meetings.
• Coordinate intake processes, provider introductions, and scheduling for staff.
