You will deliver group, individual, and family therapy to adults and geriatric patients, within a small interdisciplinary team, following physician orders and evidence-based practices. The role emphasizes thorough documentation, psychosocial assessments, treatment planning, risk assessment, and strong communication.

Qualifications

• Master’s degree in social work or related field required.
• Licensed/certified social worker or counselor (e.g., LCSW, LPC, LMFT) preferred.
• Experience with adults in PHP/IOP settings and documentation standards.

Responsibilities

• Complete psychosocial assessments for each patient.
• Complete group psychotherapy notes for each patient.
• Develop and update individualized treatment plans.
• Understand adult counseling, group therapy, and evidence-based practices.
• Maintain accurate, timely clinical documentation.
• Collaborate with psychiatrists, nurses, and case managers.
• Comply with applicable regulations and documentation standards.
