Job summary

Harris County is looking for a Guardian Case Manager to oversee and coordinate comprehensive case management for incapacitated adults declared wards. The role involves developing ward care plans, arranging housing, coordinating medical and psychiatric treatment, and maintaining meticulous case records.

Under the Adult Services Program Supervisor, you’ll ensure financial and benefits applications, conduct monthly ward visits, and manage documentation to support ward well‑being and timely

Qualifications

• Bachelor's degree in Social Science or closely related field.
• At least 1 year of case management experience with adults with disabilities.
• Proficiency with personal computer software including Outlook, Word, Excel, and case management software.

Responsibilities

• Develops, implements, and manages the Initial Ward Care Plan tailored to ward needs and functioning.
• Establishes and monitors living arrangements in the ward's best interest.
• Coordinates and consents to medical and psychiatric treatment.
• Prepares and maintains comprehensive case documentation and reports.
• Applies for public and private benefits and completes required certifications.
• Provides ongoing case management including monthly visits to wards' residences.
