Job Description

As a Case Manager, you will work collaboratively to engage, educate, communicate, and coordinate care with clients, behavioral health services, medical providers, community resources, and others to ensure that all services prescribed in the individualized treatment plan are implemented. You will perform all case management functions associated with a caseload, including outreach and follow-up, client transportation, completing assessment and service planning processes, and identifying the need for further or specialty evaluations.

Essential Duties and Responsibilities
  1. Conduct intakes, interviews, and screenings of prospective clients.
  2. Assess clients’ addiction behaviors, social needs, and other life changes.
  3. Work closely with clients to develop treatment plans and make referrals as needed.
  4. Maintain complete chart notes, uphold confidentiality, and adhere to all HIPAA policies.
  5. Facilitate and lead support groups.
  6. Attend and participate in all required agency meetings and in-service trainings.
  7. Perform other duties as assigned.
  8. Understand and implement individual treatment plans, record patient behavior, and provide a safe, supportive environment for patients.
COVID-19 Considerations

All clients and staff are required to wear face masks in group settings.

Qualifications

License/Certification
  • CPR Certification, ID, Fingerprint & clearance card.
