Job summary

Easter Seals VT, Inc. is seeking a Post Permanency Services Coordinator to empower Vermont families formed through adoption and guardianship.

You will collaborate with the State Post Permanency Manager to deliver family-centered services, including trauma-informed parenting education and advocacy, with a focus on supportive, normative adoption experiences. Responsibilities include creating care plans, writing reports, and delivering in-home services.

Qualifications

Bachelor’s degree in a related field.
Two years of experience preferred.
Excellent writing skills are required.
Valid driver’s license and reliable transportation.

Responsibilities

Write and implement plans of care for clients as approved by the consulting clinician.
Draft contact notes, monthly reports, and discharge summaries.
Provide in-home services including parenting education, trauma-informed care, and advocacy.
Assist families in finding resources and participate in treatment, school, and coordination meetings.
Attend PPSP trainings, VT Consortium trainings, and Fostering to Forever classes within the catchment area.
Visit client homes to support caregivers and ensure plan compliance.
Testify in court regarding treatment services when subpoenaed.
