Job summary

Easterseals VT is seeking a Post Permanency Services Coordinator to empower Vermont families formed through adoption and guardianship. You will work with the State Post Permanency Manager to deliver family-centered services, focusing on normative adoption experiences and early trauma recovery.

Responsibilities include developing plans of care, creating reports, and providing in-home services that address parenting education, trauma-informed care, and advocacy.

Qualifications

• Bachelor’s Degree in a related field is required.
• Two years of experience preferred.
• Excellent writing skills are necessary.
• Valid driver’s license and reliable transportation are required.

Responsibilities

• Write and implement plans of care approved by the consulting agency clinician.
• Write contact notes, monthly reports and discharge summaries.
• Provide home-based services including parenting education, trauma education, and advocacy.
• Assist families in accessing resources and participate in treatment and school teams.
• Attend PPSP trainings and VT Consortium trainings; participate in Catchment-area events.
• Visit client homes to ensure plan compliance and meet with caregivers.
• Help establish support groups for families created by adoption or guardianship and testify in court when needed.
